My practice swing is full and smooth yet my actual swing is shorter, brisker and more of a hit. I assume this must be all in the mind, so how do I get get to use my practice swing when I am actually hitting the ball?

It would be interesting to know what your self-talk is saying as you set up to take the shot. By actually writing this to me you have already created this as a belief (something you believe to be true). In order for you to play your stroke as comfortably and easily as you do when you practice you need to BELIEVE this is possible. Check in with your self-talk next time it happens and see if you have already decided the outcome??? If you consciously change your self-talk with something that is positive and forward thinking in time this will break down this belief.
TIP: Start your sentence with 'I am going to ...' this let's your sub-conscious know that what you are about to say IS GOING TO HAPPEN - so something like 'I am going to swing this ball with all the ease of a practice stroke.' You can use whatever you feel comfortable with - but you get the idea. Like any new skill this needs repeating and repeating until it becomes habit and you lose that old negative beliet that isn't serving you.
